Checking Of Prescription Orders

Rx orders: 1.

Gemfibrozil (Lopid) 750 mg tab OD. Take 1⁄2 hr after meals. Gemfibrozil (Lopid) 900 mg tab. Take once daily, 30 minutes before the evening meal.

2.

Spiriva (Tiotropium Br) 18 mcg cap. 1 cap inhaled TID with handihaler. Tiotropium Bromide (Spiriva) 18 microgram cap. Inhale 1 cap once daily with handihaler, at the same time each day.

3.

Cetriaxone Na (Rocephin) 500mg IV vial. Dosage: Adult 5 g once daily. Cetriaxone (Rocephin) 500 mg IV vial. Dosage: Adult 1-2 g once daily via slow IV injection over at least 2-4 minutes.

Augmentin 375mg cap. Dosage: Adult with mild to moderate infections 375 mg OD. Co-amoxiclav (Augmentin) 375 mg tab. Dosage: Adult with mild to moderate infections 375 mg every 8 hrs.

5.

Roxithromycin (Macrol) 300 mg tab. Take 5 mins before meals. Roxithromycin (Macrol) 300 mg tab, once daily. Take at least 15 min before meals.

6.

Sodium alginate (Gaviscon) 500mg/267mg/160mg tab. 12 y/o: take 20 mL. Sodium alginate, Sodium bicarbonate, Calcium carbonate (Gaviscon) 500mg/267mg/160mg per 10 mL oral susp. Dosage: Adult and children over 12 years old: 10-20 mL after meals and at bedtime.

Propranolol HCl (Inderal) 40 mg IV. Adult with angina 40mg BID – TID. Propranolol HCl (Inderal) 40 mg tab. Adult with angina 40 mg 2-3 times a day.

8.

Isosorbide mononitrate (Isoket 5) 5 mg tab 2 tabs sublingually for 1 dose now. Take with food. Isosorbide dinitrate (Isoket) 5 mg tab. Take 1 tab sublingually stat, 30 minutes before meals.

9.

Felodipine (Plendil ER) 2.5 mg tab now. Swallow whole, do not crush. Felodipine (Plendil ER) 2.5 mg tab stat. Swallow whole, do not crush.

10. Tramadol + Paracetamol (Dolcet) 37.5mg/325mg tab. Adult: 1-2 tab 4-6 hrly.
