1. MANUAL CASE AND POLICY The Engineering Procedure Manual (EPM) EPM is a detailed procedure for the implementation of tasks from engineering which is the technical sub division in charge, the engineering task include Implement the effectives Service Bulletin and Airworthiness Directives to the aircraft that operated by PT.DERAYA a. Develop Maintenance Program that is adopted from the manufacturing maintenance program. b. Record and control all maintenance activities, and input all record into a computerized system to maintain schedule of all maintenance activities. c. Keep all technical file, document and manual, controlling distribution all file, document and manual to other division and PT.DERAYA base. d. Issuing engineering orders if there is work to be done or can only be done by an approved maintenance organization e. Issuing engineering instruction if any service bulletin (SB) are issued by manufacture or airworthiness directives (AD) issued by local authority, analysis the method and requirement to implement SB or AD to the aircraft, Inform to technical manager and all sub ordinates if any defect and what should be done for next progress. 2. 1. BACKGROUND Airworthiness Directives (AD’s) are issued by the DGCA under the provisions of CASR Part 39 when an unsafe condition exists and that condition is likely to exist or to develop in other aircraft, aircraft engines or appliances of the same type design. AD’s mandate corrective measures that must be accomplished in a specified periods of time. These corrective measures may include one-time inspections, repetitive inspections at specific intervals, prescribed repair actions or modifications which may terminate special requirements of the A.D. To operate an aircraft, engine or appliance to which an A.D. applies, except in accordance with the requirements of the directive, is to be in “noncompliance” with the CASR’s. 2. PURPOSE The purpose of this procedures are to give the guidance for Technical services personnel of scheduling and controlling of applicable Airworthiness Directives or Service Bulletin task for each Aircraft operated by PT. DERAYA that shall be performed in the effort to maintain the continuing Airworthiness of each Aircraft. 3. RESPONSIBILITIES The technical services chief shall be responsible to set-up the maintenance task and scheduling the application of applicable AD or SB for each Aircraft operate by PT. DERAYA, Engineering PPC will coordinate with the Maintenance Manager in the scheduling of the AD accomplishments required. The Material personnel will prepare material if there are material requirements. The accomplishment of AD will be supplied/submit to DGCA by Chief Inspector. If an emergency AD is issued during other than normal working hours (i.e. nights, weekends or holidays) the DGCA will be notified by Engineering / PPC and supply them with a copy of the AD. These individuals will take whatever action is required to facilitate the accomplishment of the Emergency AD. 4. REFERENCES The AD or SB task that applicable to each type of Aircraft operated by PT.DERAYA shall be set in accordance with: Airworthiness Directives issued by the DGCA or the Authority of country of the Manufacture, or Service Bulletin issued by the Manufacture 5. COMPLIANCE AD or SB Each aircraft will have an up-to-date Airworthiness Directive Compliance Record, which will be in a form or manner that contains all of the elements of AD/SB Compliance Form. The AD Compliance summary report may be used to record either compliance of the Aircraft AD/SB or the engine AD/SB. In addition, a record system will be utilized to track Airworthiness Directives, which have a time control or repetitive compliance requirement. a. The AD Compliance Record will be kept as part of the aircraft permanent records. b. The following steps shown below illustrate the routing and review process for all incoming Airworthiness directives.  The DGCA / FAA/ EASA issues the AD.  The Chief Inspector and the Engineering / PPC receive and review the incoming AD for applicability and urgency and then distribute it to the concerned section for further review and implementation.

 Effected Department (Operations, Maintenance, etc.) will plan and route applicable AD(s) through the applicable section to obtain timely handling and accomplishment.  Completed AD(s) and associated, required paperwork are reviewed then verified by Chief Inspector for accuracy and completeness.  The completed of accomplishment AD report is forwarded to the Engineering Section where the data is transferred to the Airworthiness Directive Compliance Record  The completed AD is then filed in the aircraft records. c. AD or SB applicable to engines will be treated the same way as the Aircraft AD/SB and the necessary record(s) shall be entered in the engine logbook by Engineering Section. An AD or SB with a time control or repetitive compliance requirement will be tracked in the same manner as an aircraft AD. d. For AD or SB applicable the chief Engineering & PPC issued Engineering Instruction (EI), completed job card and work package include inspection sheet sent to Chief Line Maintenance to perform EI acknowledge by Maintenance Manager and Chief Inspector. e. Compliance of AD or SB will be recorded in the aircraft flight maintenance log and appropriate section of aircraft or engine log books. Technical record will enter all information regarding compliance of AD or SB into a computerized system which may be printed in a form of Airworthiness Directives Compliance Summary Report and ready available for inspection if SB or AD is repetitive.  For any defect found and still on limit given by manufacture Chief Engineering release EI to PPC to reschedule the maintenance Revision : ISSUE

And also sent the letter to the manufacture competed with A/C Serial Number, Time Since New, Cycle since New, and Date of work complishment  For any defect found and still on limit but need the preventive action Chief Engineering release EI to Chief Line Maintenance to take action of preventive action, Acknowledge by Maintenance Manager to scheduling repairman if any repair needed. And also sent the letter to the manufacture competed with A/C Serial Number, Time Since New, Cycle since New, and Date of work accomplishment  For any defect found and over the limit given by manufacture Chief Engineering follow the instruction state on SB or AD, if any replacement required, Chief Engineering send request the part and material needed to Material Planning Control to be forwarded to procurement department. The next step made a plan for replacement including prepare the material, man power, time frame for

scheduling

any

engineer

and

repairman.

After

the

accomplishment of replacement, Chief Engineering should sent the mail to the aircraft manufacture competed with A/C Serial Number, Time

and

Date

CHAPTER IV : MAINTENANCE PROGRAM DEVELOPEMENT

1. REQUIREMENT Technical services chief will prepare aircraft inspection and maintenance program, in accordance with the following requirements: a. Aircraft inspection and maintenance program is complete, i.e. all inspection and maintenance related publications are incorporated. b. Aircraft inspection and maintenance program is up-to-date, the program is continuously reviewed and revised to incorporate the latest revision of aircraft maintenance manual, issue of airworthiness directive or any other approved technical data. c. Aircraft inspection and maintenance program can be implemented. 2. Preparation of Aircraft Inspection and Maintenance Program The initial step in preparing an aircraft inspection and maintenance program consists of collecting all relevant data, as listed in but not limited to the following: a. Maintenance manual (airframe, engine, propeller & component) b. Illustrated Part Catalogue (airframe, engine, propeller & component) c. Maintenance Program Document d. SB / SIL (airframe, engine, propeller & component) e. AD (airframe, engine, propeller & component). Before proceeding, it should be ensured that all relevant data is up-to-date for the latest revision status.

CHAPTER IV : MAINTENANCE PROGRAM DEVELOPEMENT

The basis for the aircraft inspection and maintenance program should be the requirements prescribed in the maintenance manual of the airframe, engine, propeller and equipment manufacturer. If the airframe manufacturer recommendations differ from those of the manufacturer of the engine(s) or components, the recommendation of the airframe manufacturer shall prevail. Maintenance recommendations by service information such as service letters, service notices, service instruction etc. are not compulsory, however the Chief engineering & PPC shall review these publications and decide whether or not to include such recommendations in the aircraft maintenance program. Service information indicating a life limit (in hours, calendar time and number of cycle/landings between inspection, test and overhaul) is compulsory and shall be included in the aircraft maintenance program. The Company's Maintenance Program is a compilation of an individual maintenance and inspections.

The Maintenance Program consists of the

following elements:  The applicable of the company aircraft and the current revision of Inspection Program.  The company's aircraft Scheduled Maintenance and inspection Programs, Instructions and procedures for maintenance, preventive maintenance, and servicing (refer to CASR 121.133(b)).  The company’s aircraft Unscheduled Maintenance Program;  The company's aircraft inspection Programs for airframe and engine;  Time limitations (calendar time, operational hours, flight cycles)or standards for determining time limitations for overhauls, inspections, and

checks of airframes, engines, propellers, appliances, and emergency equipment.  The company’s aircraft Required Inspection Items Program;  Aircraft Instrument and Avionic bench test interval.  Include the maintenance task and intervals that have been specified as mandatory in approval of the type design shall be identified as such.  Procedures for aircraft refueling, elimination of fuel contamination, fire protection (including electrostatic protection), and supervision and protection of passengers during refueling (refer to CASR 121.135(c)(9)(i) and 135.135(c)(9)(i))  Instructions and procedures for maintenance personnel to follow if confronted with errors or deficiencies in documented maintenance procedures (maintenance manual, work cards, etc.), especially while performing maintenance that requires immediate corrective action to ensure safe practices and Airworthy aircraft. The procedures must include instructions for documenting the error and ensuring the deviation or corrections are validated and acceptable to the DGCA.  Airworthiness inspections, including instructions covering procedures, standards, responsibilities, and authority of inspection personnel.  Method & procedures for maintaining the aircraft’s weight & center of gravity within approved limits (ref to CASR 121.135(c)(8), 121.135(d)(7), 135.135(c)(8) & 135.135(d)(7)). 1. INTRODUCTION This chapter are describes an aircraft, aircraft engine, propeller, components and qualification personnel record system used by the PT. DERAYA The record system procedure and retention enable PT. DERAYA to achieve the maximum value and utilization of its aircraft by documenting proper parts installation a routine / non routine maintenance action. The standards for determining the time limitations of overhaul, inspection, and check of airframes, engines propeller and components including emergency equipment are documented in the maintenance program. 2. Responsibility The Chief Engineering and PPC is responsible for maintaining all maintenance record in the proper manner and in accordance with the requirement of CASR. PPC and technical publication responsible to keep all the records. 3. Maintenance Record Filing Procedures. a. All record of maintenance performance of each aircraft shall be filed in a folder. b. Each folder shall have clear identification showing the following:  The assigned number of the folder.  Description of the document filed.  The date of filing.  The retention time required c. Each folder shall have to be recorded in the file Log Book for easy to retrieve and control. 4. The following records shall be kept for the specified periods a. All the records necessary to show that all requirements for the issuance of a maintenance release under CASR Part 135 have been met. b. A description of reference to data of the work performed. c. Records of all other maintenance work that Deraya done, identifying by name and number of the licensed engineer who performed or supervised the work, and the Inspector of that work. d. Records containing the following information:  The total time in service of the airframe.  The total time in service of each engine and propeller.  The current status of life-limited parts of each airframe, engine, propeller, and appliance.  The time since last overhaul of all items installed on the aircraft which are required to be overhauled on a specified time basis.  The identification of the current inspection status of the aircraft, including the time since last inspections required by the inspection program under which the aircraft and its appliance are maintained.  The current status of applicable Airworthiness Directives and Service Bulletins including the date and method of compliance, and if the Airworthiness Directives or Service Bulletin involves recurring action, the time and the date when the next action is required.  A list of current alterations to each Airframe, Engine, Propeller, and Appliance. 5. RECORD KEEPING SYSTEM A. The Record Keeping System assures that adequate and appropriate records are maintained for all work performed on PT. DERAYA aircraft. The retention of records enables the Company to maximize the value and legislation of its assets (aircraft, engines, propeller/rotor, components, appliances, etc.) by documenting proper parts installations, and routine and non-routine maintenance actions. B. The Record Keeping System classifies all records into four retention categories to improve storage and retrieval of data. These categories are: 1) Continuous - These records are continuously updated. Such record reflects to the current aircraft operations at any points in time. 2) Routine – These records may be discarded after 2 years. Except for minor check (before departure, transit check) and AML, CML station copy must be retaining for 24 (twenty four) hours. 3) Repetitive – These records must be retained for 2 (two) years after the item or group (inspection package) is accomplished again. 4) Permanent - This category includes permanent changes to the configuration of an airplane, engine, propeller/rotor, component and appliance. Such records are retained permanently. C. The Record Keeping System assures that records will be filed and stored in such a manner that the records will be readily accessible to authorized auditors and other personnel. The filling and storage system shall promote: ease of retrieval, accessibility, and adequate controls. Technical records which are maintained in the PT. DERAYAin the hard copy and/or soft copy will be secured with adequate security systems and continuous. 6. RECORDS RETENTION Company will retain the following records as follows: a. TRANSFER RECORDS Should the ownership of the aircraft will be change to other party, all relevant original aircraft documents and records of installed components shall be transferred to the new owner at the time of transfer of title or de registration. Should the aircraft de-registration to a foreign country, an Export Certificate of Airworthiness issued by Indonesia DGCA is required. Transfer of maintenance records will take place in two cases: a. Purchase / lease in of an aircraft In the event of purchase / lease – in, assigned person will be checked and verified on behalf of Quality Department at either the aircraft owner’s location or other agreed place before the purchase / lease will take place. b. Sale / lease out of an aircraft. In the event of sale / lease-out, Technical Service will be prepare all relevant and verified by the Inspector before the sale / lease-out will take place. 1. GENERAL Technical publication in this context means all documents issued by the manufacturer of the aircraft, aircraft systems and aircraft components or by DGCA when related to technical affairs. For example, such documents are aircraft maintenance manual, flight manual, illustrated part catalogues, service bulletin, airworthiness directive, etc. It is PT. Deraya policy not to allow any person to perform maintenance, preventive maintenance and/or alterations on PT. Deraya aircraft and/or its component unless all required technical publication to support the work are available and up to date. In order to develop and maintain this situation, it must be ensured that the technical publication are at all times in good physical condition, up to date and available where and when they are required. 2. RESPONSIBILITIES a. Technical Services Chief is responsible for proper provision, control and distribution of all technical data required for company’s aircraft maintenance b. Maintenance manager is responsible to ensure that all data under his responsibility is properly maintained and utilized. All deficiencies must be reported to Technical Services & Quality Department. c. Chief Inspector is responsible to ensure that all works on company’s aircraft are performed in accordance with the approved technical data. 3. Technical Publication Control System. Due to the importance of any technical data, all incoming technical data require proper assessment. Many documents, such as revisions to existing documents may be distributed as required. It is important that the revisions

Newly received documents will be registered and introduced into the document control system. A records file will be maintained to provide adequate control of the status of all documents within the organization. b. Valid issues of the applicable documents will be available at those places where work related to the contents of the publications is carried out. Engineering section distributes the documents to those places and transmittal sheet is attached to each document forwarded with Publication Transmittal form (Form DRY-115) c. The Technical transmittal form attached last updated document, the Superseded and/or cancelled documents shall be removed from places where they are in use or to which they were issued, the removed document must be sent back to technical publication. It is the responsibility of the lead engineer or the engineer in charge of each location to ensure that the documents are kept up to date. d. Revisions shall be recorded in the relevant document and on relevant records filling system. It is the responsibility of the engineer to record the revision he had incorporated into the manual. e. Upon completion of revising the publications the engineer will fill out the transmittal sheet, publication transmittals attached to the revision and sign it off to indicate that he had incorporated and recorded the revision into the affected publications. The lead engineer or the engineer in charge is responsible to return the completed transmittal sheet to the engineering section.

f. A periodic document revision status review will be carried out in order to check validity of all copies of documents within the organization. 4. Technical Publication Status Check All technical data kept in the technical library of PT. Deraya are maintained in an up to date status at all times. It is check for current status using information published by individual manufacturer. The information is published regularly and contain information regarding revision status of all technical issued by each manufacturer. It would be the responsibility of the chief engineering to ensure that all technical data kept in the technical library of the company are up to date where as the maintenance manager or lead engineer or engineer in charge is responsible to keep the technical data in his care in an up to date status at all time. Chief engineering will maintain a Publication Status Report FORM DRY-109 sample of such report, which contains records of technical data available at a location.
