AFFIDAVIT FILED ON BEHALF OF THE PETITIONER U/S 297 Cr.P.C I, Atchukatla Anjamma W/o.late Narasaiah, aged 35 years, Hindu, resident of D.No.3-48, S.C. Colony, Singannapalem Village, Mundlamuru Mandal, Prakasam District, do hereby solemnly affirm

and sincerely state as follows:I am a 3rd party to this proceedings and petitioner herein.

I

know the facts of the case. I submit that we filed a petition before the Prl. District Judge Court, Ongole, claiming compensation for the death to my Husband, who died in a motor accident.

The Police filed the

Charge Sheet against the Driver of the Lorry bearing No.AP 16 TB 9577, who caused the accident and all the necessary documents are filed into this court. I am advised to file the certified copies of F.I.R, Inquest Report, Post-Mortem Certificate, M.V.I. Report, Rough Sketch, and Charge Sheet and to prove the negligence of driver and also my case.

The documents

mentioned in the Copy Application are very essential and necessary to prove the contents of the O.P. Since I am a 3 rd party to these proceedings, I need permission of this Hon’ble Court to obtain the certified copies. Hence I am filing this application. It is therefore prayed that this Hon’ble Court may be pleased to permit me to obtain the certified copies mentioned in the copy application and pass necessary orders in the interest of justice. Else, I will be put to irreparable loss and injustice.
