EMERGENCY PREPAREDNESS MECHANICAL DYNAMICS INC. C2016

Disaster can occur anywhere, and they often occur when we least expect them. The potential for emergencies and disasters to happen at all plant facilities, even at home can be devastating in terms of casualties, business interruptions, loss of capital investments, etc. These events cannot be avoided but our management can reduce their frequency of occurrence and severity of damage when effective preparation and planning is done. No matter how committed an industrial company may be in terms of occupational safety and health program implementation, the probability of an emergency may arise at the least expected times. For that, we made this manual to help employees make decisions and take appropriate actions to keep everyone as safe as possible in emergency cases. The objective of this manual is to: • • • •

COMMUNICATIONS In the event of an emergency in our country, people rely on televisions and radio. But for our company, we will be informed by alarm systems that sounds like a bell ringing continuously. A speaker phone will also be used to warn everyone about the said emergency situation. Cellphones are used for communication, but in cases where there will be no signal from outside networks, two-way radio system will be

used by assigned personnel. If the situation cannot be handled by the company itself, the assigned personnel will communicate outside agencies to respond as soon as possible.

SPECIAL DUTIES First Aiders (FA) One who gives immediate care to a victim who has been injured or suddenly ill. Serves as the bridge that fills the gap between the victim and the physician. Will respond immediately to the scene once alarmed. Fire Extinguisher Squad (FES) Responds to small fires in their designated area. When the fire can’t be handle anymore, FES will notify the fire hose team (FHT) and the Security & Communication Team (Guards on duty) to warn the outside fire departments. Fire Hose Team (FHT) They will try to cease the fire until the fire fighters from outside arrives. Attendance Checker (AC) To know if someone is missing, AC’s will check the attendance of their assigned areas and will report immediately to the Predesignated Assembly Point Captain (PAP-C). Electrical Squad (ES) ES are responsible to close the circuit breaker as ordered by the Building Captains (BC). Floor Warden (FW) Floor Wardens are responsible for the immediate response to emergency situations. Also, they are the leaders of evacuation for their respective areas. They should communicate with the BC’s. Security and Communications Team (Guards) The security and communications team will communicate within the company or warn outside agencies about the emergency in the facilities. They are also responsible to handle bomb threat cases and will coordinate all the time with the BC’s and the PAPC.

PAP Captain (PAP-C) On our Pre-designated Assembly Point (PAP), the PAP-C will be the one who will supervise and give orders to everyone in that area.

DUTIES & RESPONSIBILITIES: 1. Show bravery amongst all. 2. Ensure that evacuees will be in order and in their respective areas in the PAP. 3. Coordinates with the FLOOR WARDENS about the status of the evacuation. 4. Checking the attendance: a. The Security Guard will give the PAP-C the attendance sheet folder. There, you will see five attendance sheets, and a blank paper for scratch. The ATTENDANCE CHECKERS will go to you, and you will give them the copies for each group. (Office, Warehouse, Fabrication 1, Fabrication 2, Visitors) b. After some minutes, the ATTENDANCE CHECKERS will report the status of their groups. If one of the CHECKER told the PAP-C that there is someone missing from his/her group, double check it with the logbook of the security guard. If he/she is not absent from the guard’s logbook, it means that he/she is still inside the building. c. If there is a missing person from the group, coordinate with the SEARCH & RESCUE TEAM. The head of the SRT is the BUILDING CAPTAIN of the Office. 5. Monitor the injured employees in the first aid area. 6. Give orders to the Security Guard if needed to call outside agencies like Bureau of Fire and Protection (BFP), Philippine Red Cross, and other agencies. 7. Give orders if the evacuation has ended already, and the building is already secured and still good to work in. The status of the building will be given by the SRT. Building Captain/s (BC) Before the evacuation going to the Pre-designated Assembly Point, the head for any emergency inside the company premises are the Building Captains. DUTIES & RESPONSIBILITIES: 1

Should have clear communications with the FLOOR WARDENS, SECURITY & COMMUNICATIONS TEAM and another BUILDING CAPTAIN regarding any incident in our company.

Will respond to an emergency that FLOOR WARDENS can’t handle. Give order if evacuation of the building is needed. Together with the FLOOR WARDENS, ensures everyone have evacuated already. If they encounter injured employees during evacuation, they should help them and communicate with the Predesignated Assembly Point Captain (PAP-C) to call for first-aiders if really needed. 5 Should have clear communication with the PAP-C during evacuation. 6 If there will be employees left inside the building, the Building Captain of the Office will lead the Search and Rescue Team. 7 Help the outside agencies like Bureau of Fire Protection (BFP) if they will be called to respond. The Security & Communications Team will contact them, given by the order of the PAP-C, which is communicated with the Building Captains. 8 Assess the building and the situation if already under control. 9 Advice the PAP-C to give further information regarding the assessment. 10 The PAP-C will be the one to relay to every evacuee about the status of the building and will decide if employees can go back already to the building.

Search & Rescue Team (SRT) Together with outside agencies, the search & rescue team will work hand in hand to search & rescue victims that are still inside the company facilities.

GENERAL EMERGENCY PLAN In situations like fire, hazardous material exposure, transportation accident, machine accident, and the like, we have witnesses. If you are the witness, what should you do next? 1. DON’T PANIC. 2. Evaluate if you can respond to that situation: a. YES. Then tell someone closest to you about the situation and go for it. The person notified by the witness should immediately report it to the floor warden (FW). If there is no one near you, just go for it. If you solved it already, well done, but if it just got worse, report it to the floor warden (FW). b. NO. Don’t be a superhero, just report it immediately to the floor warden (FW). 3. The floor warden (FW) will: a. HANDLE THE SITUATION. They are trained.

b. NOTIFY OTHER SPECIAL DUTIES (FA, FES). c. NOTIFY THE BUILDING CAPTAIN (BC). If evacuation procedure is needed, the FW will command someone nearest to him/her to notify the building captain. The building captain is responsible to notify all personnel and visitors when it is necessary to evacuate the facility/building.

EVACUATION PROCEDURES 1. DON’T PANIC and listen for information or instructions. 2. Employees seated near windows shall secure that windows are close. Don’t jump out of the window, unless eagerly needed. 3. Any employee next to a restroom or a vacant room shall check that room and, if anyone is there, instruct that person(s) to proceed to the predesignated assembly point. 4. Employees are not to take anything with them unless ordered by their boss for important documents or in case of fire. 5. Floor wardens will be the last person to leave in their assigned areas, ensuring everyone to be evacuated first. 6. The Pre-designated Assembly Point (PAP) are sidewalks of the road outside the MDI area. 7. Employees should move quickly and safely as possible without creating more hazards than already encountered. a. GATES: i. Gate 1 – Gate at the back of Fab 1 ii. Gate 2 – Gate near parking area iii. Gate 3 – Gate going to MDI Bldg. 3 8. Routes: a. For MDI Building 1 Office. Take the lobby entrance going to Gate 2. b. For MDI Building 1 Fab. You can take the exit located at the left side (where delivery materials are released) going to Gate 1, or the right side entrance going to Gate 2. c. For MDI Building 2 Machine and Hot Works Area. You can go directly to Gate 2, or take the improvised ladder going to Gate 3. 9. If primary exits are blocked, for any reason, then go to alternative exits. a. For MDI Building 1 Office Accounting & Purchasing Dept. If the lobby entrance is blocked, take the second floor route to executive office going to the fire exit door and going out to Gate 2. For HR Dept., you can take the warehouse entrance door. 10.If a fire is in the corridor just outside the room or close enough to the exit route, then close the door block all gaps around the door until a move can be decided. 11.Attendance in each area should be checked by Attendance Checker (AC), and should report directly to the Pre-designated Assembly Point Captain. 12.Pray!

CONSIDERATION FOR EMERGENCIES WHAT TO DO DURING AN EARTHQUAKE? 1. DROP, COVER & HOLD 2. Act quickly! Protect your body from falling debris by getting under a sturdy desk, table or doorway. 3. Keep away from glass windows and free standing furniture. 4. When outside, stay away from tall equipment, move to an open field or go immediately to PAP. 5. At the end of initial shock, evacuate building/fabrication when given clearance by the BC. Follow the evacuation procedures said earlier. 6. Go to the Pre-designated Assembly Point (PAP). WHAT TO DO DURING A BOMB THREAT? 1. If a bomb or suspicious object is discovered, do not touch it, clear the area. 2. Notify the Security and Communication Team and the Building Captain. 3. Prevent other personnel from going into the area. 4. The security and will take any other action required. 5. If evacuation is needed, he will notify the Building Captain and the evacuation procedure will be followed. 6. If you received a call for a bomb threat, a. Attract attention of another staff member to ring your local hotline. b. Record details of threat as per procedure given: do not hang up your phone. c. Try to record exact words; keep the caller talking and try to obtain as much information as possible. d. Security will take further action required. e. QUESTIONS TO ASK: i. When is the bomb going to explode? ii. Where is it right now? iii. What does it look like? iv. What kind of bomb is it? v. What will cause the bomb to explode? vi. Who place the bomb? vii. Why? viii. Where are you? ix. What is your name? DO NOT HANG UP THE PHONE
