1. Referral Question 2. Behavioral Observation (2 paragraphs) BO+ MSE (during the WAIS) 3. Cognitive Functioning Section (7 paragraphs)

▪ Paragraph 1--what is WAIS i. What is the purpose : measure of cognitive functioning; ii. \What are the norms: mean, standard deviation for the entire thing and the subtests--Normed by ages, ethnicity, religions iii. Domains it assesses SAMPLE: Mr. T was administered a Wechsler Adult Intelligence Scale–Fourth Edition (WAIS IV), which is a test that measures intellectual functioning and cognitive abilities. The WAIS IV is normed by age, using stratified sample that considers ethnicity, sex, education, religion, is divided into 4 indexes that each assesses specific areas of cognitive functioning. They are verbal comprehension (VCI), perceptual reasoning (PRI), working memory (WMI), processing speed (PSI). Index is integrated into a full scale intelligence quotient FSIQ SD=15 standard scores with a mean score of 100; Scales falls on subtests that takes place in each indices have a mean of 10 and standard deviation of 3. Analyses on performance in each index will provide understanding of his cognitive strengths and weakness relavent to same age peers. • Paragraph 2--FSIQ A. B. i. ii.
