INTRODUCTION This Confine Space Plan serves to outline the safe operating specifications as well as procedure and guidance required for project related to Occupational Safety and Health Requirements for confined space.

2.0

SCOPE OF APPLICATION This Confine Space Plan shall be applied to all activities in confine space that related to MRT SSP Package V203.

The employer shall determine if his facility has any confined space, and if so, develop and maintain a record of confined spaces. If a place of work has a confined space, the employer shall inform his employees and any other persons by posting danger signs or by any other equally effective means of the existence and location of the confined space and the danger posed by them, especially when work is being executed. A sign reading “DANGER --- CONFINED SPACE. DO NOT ENTER” or using other similar meaning and in both Bahasa Malaysia and English would satisfy the requirement for a sign and shall be clearly visible. If the employer decides that his employees have to enter a confined space, the employer shall develop and implement a confined space entry programme. This programme shall be documented and made available to the employees. If the employer decides that his employees shall not enter a confined space, the employer shall take effective measures to prevent his employees and any other persons from entering the confined space.

RESPONSIBILITIES The Person in Charge is responsible to ensure the requirements of this safe procedure on working in confined space as well as general Safety and Health requirements are effectively implemented, monitored and reviewed at workplace. To monitor the implementation, the Project Team shall be responsible to supervise, establish and document specific records for this activity. Entering and/or working in a confined space shall only be carried out with the presence of competent confined space team which consist of Team Member Permit Issuer

Responsibility  Identify all hazards and recommends control measures to ensure safe entry;  Ensure there is no other incompatible activity in the vicinity of the confined space;  Authorize the permit to work; and  Endorse the cancellation and closure of the permit to work  Supervise all the activities before, during and after entry operation  Verify by checking that the appropriate entries have been made on the permit to work, all tests specified by the permit to work have been conducted and that procedures and equipment specified by the permit to work are in place before the permit issuer authorize the permit to work and allow entry to begin.  Terminate the entry and cancel the permit to work if the acceptable entry condition cannot be maintained.  Close the permit to work at the completion of the entry.

Responsibility Competency  Verify that rescue services are available before any entry and that the means for summoning them are operable;  Remove unauthorized individuals who enter or who attempt to enter the confined space  Brief the entry team on the hazards, permit to work requirement and emergency procedures  Identify all chemical hazards and recommends control measures to ensure safe entry  Has passed Authorized Entrant Standby Person & Authorized  Be familiar with all confined space entry equipment used, for example emergency Gas Tester – Entry Supervisor breathing apparatus, man riding winch, tripod, etc examination.  Test and interpret the reading level of gaseous in the confined space.  Registered with DOSH

Monitor activities outside and inside of every confined space to determine if it is safe for entrants to remain in the confined space and orders the authorized entrants to evacuate a confined space immediately under any of the following circumstances – i. if the stand-by person detects a prohibited condition ii. if the stand-by person detects the behavioral effects to any authorized entrant due to exposure to hazard iii. If the stand-by person detects a situation outside the space that could endanger the authorized entrants iv. If any alarm is activated.  Summons rescue and other emergency services as soon as he determines that authorized entrants may need assistance to escape from confined space Monitor the condition of equipment required for safe entry work such as lighting and ventilation Authorized Entrant

CRITERIA OF CONFINED SPACE Confined space means an enclosed or partially enclosed space that is at atmospheric pressure during occupancy and is not intended or designed primarily as a place of work, and is liable at any time to i. has an atmosphere which contains potentially harmful levels of contaminants ii. have an oxygen deficiency or excess iii. cause engulfment A confined space could also have restricted means for entry and exit. The following are some examples of confined spaces i. Storage tanks, tankers, silos and other tank like compartment usually having a manhole for entry ii. open-topped spaces such as pits or degreasers; iii. pipes, sewers, tunnels, shafts, ducts and similar structures. iv. Man, sized ducts and large diameter piping v. Segmental box girders (more than 7 SBG sections). The following are some examples of the activities in a confined space i. cleaning of sludge and other waste materials ii. inspection of the physical integrity of process equipment iii. maintenance, including abrasive blasting and application of surface coatings iv. repair, including welding, modification and adjustments to mechanical equipment v. rescue of workers who are injured or overcome inside the confined space

7.0

PERMIT TO WORK All confined space activities shall obtain ‘Permit to Work’ as per HSE-CDF-043 from PERMIT ISSUER authorized by IJM Person in Charge before commencing work. Whenever Permit To Work from authority is required, the permit shall be obtained first before Permit To Work from Permit Issuer can be issued out. The approved permit to work shall be made available at the time of entry to all authorized entrants, by posting it at the entry portal. The ‘Permit to Work’ for working in a confined space is valid for maximum twelve (12) hours only. The permit issuer shall terminate entry and cancel entry permit to work when a condition that is not allowed under the Permit To Work arises in or near the confined spaces. The Permit To Work shall be submitted for close out by entry supervisor after all works are completed. Permit Issuer shall close out the Permit to Work which shall be acknowledged by IJM Person in Charge.

8.0

ISOLATION All potentially hazardous services should be isolated prior to any person entering the confined space. For electrical sources, lock-out tag-out system shall be implemented.Safety devices shall be removed only by the person who originally installed them.

SAFETY MEASURE FOR CONFINED SPACE Where the presence of a confined space exists: i) A record of the description and the location of the confined space must be established. ii) Persons who may be liable to be exposed to the hazards of the confined space must be informed of the existence of the hazards. Warning signs must be clearly and visibly displayed at every access point to a confined space to warn persons of the existence of the confined space and to prevent unauthorized access. All openings into a confined space where the risk of fall of persons or materials exist must be effectively protected. Adequate and suitable lighting must be provided for any entry into and work in a confined space. Adequate and effective ventilation in a confined space must be provided and maintained at all times for the purposes of entry into and work in the confined space. The air supply for the ventilation must be:  From a source, free from contaminants  Directed to the area where a person is or will be present in the confined space. Oxygen shall not be used to ventilate a confined space. Oxygen enrichment (excess) of an atmosphere can result in increased flammability level. Safe ingress and egress for authorised entrants shall be provided. Workers shall also be equipped with emergency and rescue equipment.

10.0

REQUIREMENT TO ENSURE SAFETY OF ATMOSPHERE No person shall enter a confined space until the level of exposure is brought down below the permissible exposure limit and all risks from every hazard are reduced. Precautions shall be implemented to establish and maintain a safe breathing atmosphere in a confined space. The following action shall be done: a) Initial cleaning b) Purging c) Testing and evaluation of the atmosphere in the confined space d) Retest and/or monitoring e) Ventilation Where practicable, all solids and liquids, which are liable to present a hazard to persons inside the confined space, shall be removed from the confined space prior to entry of such person Where necessary, the confined space shall be cleared of contaminants by use of a suitable purging agent. Care shall be taken in the purging of a confined space to prevent collapse of the enclosure due to pressure differentials Ventilation shall be provided continuously throughout the period of occupancy in the confined space Continuous monitoring of the atmosphere is required either by Confined Space Entrants using personal monitoring devices or by installing fixed monitoring system that incorporates an automatic alarm.

Evaluation of a confined space conditions shall be conducted as follows:  Pre-entry check  During entry check 10.1

During pre-entry check:

The atmosphere shall be tested by an authorized gas tester When testing for atmospheric hazards, the first test is for oxygen, followed by flammable gases and vapors and finally for toxic gases and vapors. Testing and evaluation of the atmosphere of other hazards shall be performed from outside the confined space, before any entry occurs. The results shall be recorded on the permit to work;

During entry check:

Entry conditions shall be continuously monitored in the areas where authorised entrants are working. Assess whether there is any hazard from hazardous atmospheres and extremes of temperature; and If a hazardous atmosphere is detected during entry or during work in the confined space – a. Every employee shall leave the space immediately; b. Entry supervisor shall cancel the permit to work immediately and review the condition; c. The space shall be evaluated to determine how the hazardous atmosphere has developed; and d. Measures shall be implemented to protect employees from the hazardous atmosphere before any subsequent entry takes place.

All precautions above shall follow the specifications of Industry Code of Practice for Safe Working in a Confined Space (ICOP) 2010. In the event of the atmosphere inside the confined space could not be brought to safe working conditions and entry is deemed required, the authorised entrant entering into hazardous atmosphere shall use suitable self-contained breathing apparatus (SCBA), compressed airline breathing apparatus or special life supporting system depending on the nature of works and hazards involved. Provisions and procedure for rescue and emergency services shall be established and maintained in the Project Emergency Response Plan. Authorised entrant intending to work in confined space shall be certified physically and mentally fit determined by an occupational health doctor. Health fitness certificate issued by the occupational health doctor shall be recorded and kept by the employer. Authorized entrant shall declare that he is fit to enter the confined space in the Confined Space Permit to Work. (Medial Report)
