SCOPE This procedure applies to all works to be executed in a confined space that pose a risk to employees. A confined space is a place which is substantially enclosed (though not always entirely), and where serious injury can occur from hazardous substances or conditions within the space or nearby (e.g. lack of oxygen).

1.1

Purpose The purpose of this procedure is to:  Outline the requirements for practices and procedures to protect employees from the hazards of entry into permit-required confined space.  Establish a comprehensive system that protects employees from a wide variety of potential hazards while working in confined spaces.  Ensure compliance with relevant legislation.  Put in place adequate emergency arrangements before the work start.  Require all SUBCONTRACTORS, Lower Tier SUBCONTRACTORS, Vendors and all other professionals taking part in the Project to carry out a suitable and sufficient assessment of the risks for all work activities for the purpose of deciding what measures are necessary for safety. For work in confined spaces this means identifying the hazards present, assessing the risks and determining what precautions to take.

1.2

Project Description The Project consists in the Engineering, Procurement and Construction (EPC) for Al Dabb‟iya Phase Facilities Development: CENTRAL PROCESSING PLANT, GATHERING SYSTEM, EXPORT PIPELINE (OIL and GAS), FLARE SYSTEM, WATER SUPPLY and DISPOSAL, in Dabb‟iya area (Southwest of Abu Dhabi).

DEFINITIONS AND ABBREVIATIONS Refer for Definitions and Abbreviations to the Project Communication Procedure between ADCO and CONTRACTOR, appendix 1 or to the Contractual Document: General Scope of Work - Doc. No. 02.99.91.0609 Rev 3 - Paragraph 4Terminology. For the purposes of this procedure, the following definitions / abbreviations shall apply:

ACCEPTABLE ENTRY The conditions that must exist in a permit space to allow CONDITIONS entry and to ensure that employees involved with a permitrequired confined space entry can safely enter into and work within the space. ADCO Area Manager/Authority

An ADCO Divisional Manager appointed by ADCO General Management with responsibilities for a specified area, asset or project.

ADCO Issuing Authority An ADCO employee who is a direct subordinate of the Area Authority and who is authorized in writing by the Area Manager following satisfactory completion of the PTW Certification and Approval Assessment process. He authorizes the job to proceed after checking and certifying that the worksite is safe within the BROWNFIELD Areas and GREENFILED Areas which have not been handed over to CONTRACTOR. Watcher or Standby Person

monitors the authorized entrants and who performs all attendants‟ duties assigned in the employer's permit space program. He is competently trained in Confined Spaces Entry Course. He cannot leave his watch location at the entrance of confined space for any reason.

AUTHORIZED ENTRANT

It is an employee who is authorized by the employer to enter a permit space.

BLANKING or BLINDING

The absolute closure of a pipe, line, or duct by the fastening of a solid plate (such as a spectacle blind or a skillet blind) that completely covers the bore and that is capable of withstanding the maximum pressure of the pipe, line, or duct with no leakage beyond the plate.

CONFINED SPACE It is an ADCO certificate which must be issued, regardless ENTRY CERTIFICATE of location, for any confined space entry permit request. (CSEC) Examples:  Entry into any confined space within ADCO area of operation such as a vessel, tank, furnace, sewer, pit over one meter deep and other confined spaces. FST consider as Confined Space and needs entry permit to enter as its height will be more than one meter;  Entry into an excavation over two meters deep with a width to depth ratio less than 2; CONFINED SPACE

A CONFINED SPACE is a space that:  Is large enough and so configured that an employee can bodily enter and perform assigned work, and  Has limited or restricted means for entry or exit (e.g., tanks, vessels, silos, storage bins, hoppers, vaults, and pits are spaces that may have limited means of entry), and  Is not designed for continuous employee occupancy. Examples: Any space where the access or egress is limited, where oxygen deficiency may occur, 19.5% oxygen or below or an oxygen enriched atmosphere in excess of 23.5% oxygen by volume, or where toxic substances or other hazards may accumulate in a restricted work area. Confined spaces include tanks, vessels, hoppers, bins, tank cars, steam boilers, conveyor tunnels, coal bunkers, sumps, excavations below 1.2m or 4 feet (according to OSHA), ducts, scrubbers, manholes, sewers etc. A confined space includes any chamber, tank, vat, pipe, or similar confined space.

CONTRACTOR Issuing A CONTRACTOR Supervisor/employee who is a direct Authority subordinate of the CONTRACTOR Site Manager and who is authorized in writing by the CONTRACTOR Site Manager following satisfactory completion of the PTW Certification and Approval Assessment process. He shall verify the compliance with applied PTW procedure and its effectiveness. Within the GREENFIELD Areas, which have been handed over to CONTRACTOR [Open Areas - CONTRACTOR PTW(s) applied], the CONTRACTOR Issuing Authority authorizes the job to proceed after checking and certifying that the worksite is safe. Within the BROWNFIELD Areas the CONTRACTOR Issuing Authority will grant its approval to the ADCO PTW(s) by countersigning in addition to the ADCO Issuing Authority for activities to be realized by the SUBCONTRACTOR. Only when approved/signed by the CONTRACTOR Issuing Authority, then the ADCO PTW(s) can be submitted to the ADCO Issuing Authority for the final approval. DOUBLE BLOCK AND The closure of a line, duct, or pipe by closing and locking BLEED and tagging two in-line valves and by opening and locking or tagging a drain and vent valve in the line between the two closed valves. EMERGENCY

It is any occurrence (including any failure of hazard control or monitoring equipment) or event internal or external to the permit space that could endanger entrants.

Employee

Any person engaged in activities for the benefit of the Project or CONTRACTOR (SUBCONTRACTOR) and who receives payment, even on a temporary basis. This includes so called Day Labourers employed by the Project or CONTRACTOR, SUBCONTRACTORS and LOWER TIER SUBCONTRACTORS

Employment

All works or activities performed in carrying out an assignment, or request of the Project, or CONTRACTOR, SUBCONTRACTOR, including related activities not specifically covered by the assignment or request. This includes driving to and from a workplace if Project related.

ENTRY

It means the action by which a person passes through an opening into a permit-required confined space. Entry includes ensuing work activities in that space and is considered to have occurred as soon as any part of the entrant's body breaks the plane of an opening into the space.

ENTRY PERMIT

It means the written or printed document (refer to as “PERMIT”) that is provided by the responsible party (ADCO or CONTRACTOR Issuing Authorities) to allow activities and control entry into a confined space.

ENTRY SUPERVISOR For the purpose of this procedure, the Entry Supervisor is a CONTRACTOR Issuing Authority who, because of qualifications and experience, has the skills necessary to perform and to carry out risk assessment for work in

confined space. The Entry Supervisor is the person responsible for determining if acceptable entry conditions are present at a permit space where entry is planned, for authorizing entry and overseeing entry operations, and for terminating entry. He is competently trained in Confined Spaces Entry Course. GREENFIELD Area

Areas that don‟t have any existing facilities within (e.g.: new Clusters, new CPP area, etc.).

HAZARDOUS ATMOSPHERE

An atmosphere that may expose employees to the risk of death, incapacitation, and impairment of ability to selfrescue (i.e., escape unaided from a permit space), injury, or acute illness from one or more of the following causes:  Flammable gases, vapour, or mist in excess of 10%of its lower flammable limit (LFL).  Airborne combustible dust at a concentration that meets or exceeds its LFL [Note: This concentration may be approximated as a condition in which the dust obscures vision at a distance of 5 feet (1.52 m) or less].  Atmospheric oxygen concentration below 19.5% or above 23.5%.  Atmospheric concentration of any substance which could result in employee exposure in excess of its dose or permissible exposure limit. [Note: An atmospheric concentration of any substance that is not capable of causing death, incapacitation, impairment of ability to self-rescue, injury, or acute illness due to its health effects is not covered by this provision].  Any other atmospheric condition that is immediately dangerous to life or health.

HOT WORK PERMIT

The employer's written authorization to perform operations (e.g., riveting, welding, cutting, burning, and heating) capable of providing a source of ignition.

IMMEDIATELY Any condition that poses an immediate or delayed threat to DANGEROUS TO LIFE life or that would cause irreversible adverse health effects or AND HEALTH (IDLH) that would interfere with an individual's ability to escape unaided from a permit space. [Note: Some materials (hydrogen fluoride gas and cadmium vapor, for example) may produce immediate transient effects or that, even if severe, may pass without medical attention, but are followed by sudden, possibly fatal collapse 12-72 hours after exposure. quantities are considered to be "immediately" dangerous to life and health]. Job Originator (ADCO) An ADCO employee who is competent in the ADCO PTW system as a Job Performer (by passing ADCO course entitled "PTW Job Performer”) and who signs the Work Permit request confirming the requirement to carry out the work. The Job Originator approval/signature on PTW is only required in case JP is a CONTRACTOR or SUBCONTRACTOR; the JO must be an ADCO supervisor or an ADCO representative (e.g. PMC in Project) Job Performer (SUBCONTRACTOR)

A SUBCONTRACTOR employee who is authorized in writing by the SUBCONTRACTOR Site Manager, following satisfactory completion of the ADCO PTW Certification course entitled “PTW - Job Performer” and Approval Assessment process and to whom the PTW/Certificate is issued. The Job Performer is directly responsible for the quality and safety of the work to be performed and for conducting a Toolbox Talk (TBT) immediately before commencing the work.

INERTING

It is the displacement of the atmosphere in a permit space by a non-combustible gas (such as nitrogen) to such an extent that the resulting atmosphere is non combustible. [Note: This procedure produces an IDLH oxygen-deficient atmosphere].

ISOLATION

It means the process by which a permit space is removed from service and completely protected against the release of energy and material into the space by such means as: blanking or blinding; misaligning or removing sections of lines, pipes, or ducts; Lock-Out & Tag-Out of all sources of energy; or blocking or disconnecting all mechanical linkages.

LINE BREAKING

The intentional opening of a pipe, line, or duct that is or has been carrying flammable, corrosive, or toxic material, an inert gas, or any fluid at a volume, pressure, or temperature capable of causing injury.

NON-PERMIT CONFINED SPACE

A confined space that does not contain or, with respect to atmospheric hazards, have the potential to contain any hazard capable of causing death or serious physical harm.

OXYGEN DEFICIENT ATMOSPHERE

It is an atmosphere containing less than 19.5% oxygen by volume.

OXYGEN ENRICHED ATMOSPHERE

It is an atmosphere containing more than 23.5% oxygen by volume.

PERMIT-REQUIRED CONFINED SPACE (PERMIT

Has an internal configuration such that an entrant could be trapped or asphyxiated by inwardly converging walls or by a floor, which slopes downward and tapers to a smaller cross section. d. Contains any other recognized serious safety or health hazard.

PERMIT-REQUIRED CONFINED SPACE PROGRAM

CONTRACTOR's overall program for controlling, and where appropriate, for protecting employees from, permit space hazards and for regulating employee entry into permit spaces.

PERMIT SYSTEM

The employer's written procedure for preparing and issuing permits for entry and for returning the permit space to service following termination of entry.

PROHIBITED CONDITION

Any condition in a permit space that is not allowed by the permit during the period when entry is authorized

RESCUE SERVICE

The personnel designated to rescue employees from permit spaces.”

RETRIEVAL SYSTEM

Equipment (including a retrieval line, chest or full-body harness, wristlets, if appropriate, and a lifting device or anchor) used for rescue of persons to be worn within a confined space which allows the worker to be tied off and safety connected when working underground or in difficult to access areas.

TESTING

The process by which the hazards that may confront entrants of a permit space are identified and evaluated. Testing includes specifying the tests that are to be performed in the permit space. [Note: Testing enables employers both to devise and implement adequate control measures for the protection of authorized entrants and to determine if acceptable entry conditions are present immediately prior to, and during, entry].

ROLES AND RESPONSIBILITIES CONTRACTOR Construction Director (CD) The CONTRACTOR CD has the overall responsibility for this procedure and is responsible for supporting it and for ensuring that the jobsite under his responsibility actively participate. The CONTRACTOR CD is responsible for providing the personnel, facilities, and other resources necessary to effectively implement this procedure availing of specific cooperation of the Site Manager(s) for respective area of competency. CONTRACTOR Site Manager It shall be the responsibility of the CONTRACTOR Site Manager to ensure that the requirements of this Procedure are adhered to. SUBCONTRACTOR Site Manager It shall be the responsibility of the SUBCONTRACTOR Site Manager to ensure that the requirements of this Procedure are adhered to. In addition, the SUBCONTRACTOR Site Manager:  Evaluates the facilities and assigned site areas within scope of work for confined space hazards and installs warning signs at all entrances.  Ensures that safe working condition requirements have been achieved for the confined space entry permit.  Ensures that all personnel involved with the confined space entry have been trained in accordance with confined space entry requirements.

 Ensures that all employees who enter a confined space have received the necessary information on respect to the work to be performed.  Provide prescribed protective equipment for confined space entry operations. CONTRACTOR/SUBCONTRACTOR Supervisor(s) It shall be the responsibility of all CONTRACTOR‟s and SUBCONTRACTOR‟s Supervisors to ensure that all employees adhere to the requirements of this Procedure. ADCO Area Safety Officer [as defined in the ADCO PTW Procedure]  The ADCO Area Safety Officer advises and is consulted by the Area Authority for Confined Space Entry Certificates, Area Permits and high-risk jobs. Job Performer (SUBCONTRACTOR)  Maintain a record of personnel entering and leaving the space. CONTRACTOR Emergency Response Coordinator (ERC) The CONTRACTOR ERC performs the following tasks:  Assists the CONTRACTOR Site HSE Manager in evaluating the capabilities and qualifications of offsite emergency/rescue personnel.  Where required, participates in Confined Space planning where restricted access or egress exists.  Ensures that site emergency rescue personnel and equipment are available during confined space permit activity.  Plans and executes emergency confined space rescue drills. CONTRACTOR Entry Supervisor The Entry Supervisor is the person responsible for determining if acceptable entry conditions are present at a permit space where entry is planned, for authorizing entry and overseeing entry operations, and for terminating entry. He is competently trained in Confined Spaces Entry Course. In addition of the CONTRACTOR Issuing Authority duties stated in the PTW procedure (3844-LZ-PC-GA000107), the Entry Supervisor performs the following tasks:  Confirms, by checking, that the appropriate accesses have been made available as stated on the PTW, that all tests indicated in the permit have been conducted and that all procedures and equipment indicated by the permit are in place before issuing the permit and allowing entry to begin.  Verifies that all pre-entry tests specified by the permit have been conducted and that acceptable entry conditions exist.  Ensures that all procedures and equipment specified by the permit are in place prior to entry.  Endorses the permit and authorizes entry.  Terminates the entry and cancels the permit as necessary.  Verifies that rescue services are available and that the means for summoning them are in place and operable.  Removes unauthorized individuals who enter or attempt to enter the space during entry operations.

 Determines that entry operations remain consistent with terms of the permit and that acceptable entry conditions are maintained.  Ensures safe closure and maintains control of the confined space.  Prevents unproved individuals to enter the permit space during entry operations  If an entrant has the potential to be exposed to a substance for which a Material Safety Data Sheet or COSHH sheet is required to be kept at the worksheet, the written information shall be made available to the medical facility treating the exposed entrant. NOTE: the CONTRACTOR Site Management shall ensure that each Entry Supervisor is aware of the hazards that may be encountered during entry, including information on the mode, signs or symptoms and consequences of exposure. Confined Space Watcher/Standby Person (SUBCONTRACTOR) The Confined Space Watcher/Standby person performs the following tasks:  Continuously maintains an accurate record of authorized entrants.  Remains outside the permit space during entry and/or rescue operations until relieved by another attendant.  Do not enter the confined space under any circumstances  Communicates with authorized entrants as necessary to monitor entrant status and to alert entrants of the need to evacuate the space.  Know existing and potential hazards, including information on the mode of exposure, signs or symptoms, consequences of the exposure, and their physiological effects  Monitors activities inside and outside the space to determine if it is safe for entrants to remain inside the space or non-entry personnel to remain outside the space nearby.  Orders evacuation under the following conditions: o Upon detection of a prohibited condition o Upon detection of the behavioral effects of hazard exposure to authorized entrants o Upon detection of a situation outside the space that could endanger the entrants o Upon noting that the attendant cannot effectively or safely perform required duties  Summons rescue/emergency services upon determination that authorized entrants may require assistance to escape from the space.  Takes the following actions when unauthorized persons approach or enter the space: o Warns the unauthorized person to stay away o Advises an unauthorized person to exit the space o Advises authorized entrants and the Entry Supervisor that an unauthorized person has entered the space

 Performs no duties that might interfere with the primary duty – monitoring and protecting authorized entrants.  Is competently trained in Confined Spaces Entry Course.  Have immediately available emergency rescue equipment on site and personal protective equipment the same as the entrant.  Inform authorized entrants and Entry Supervisor of entry by unauthorized persons  Be familiar with the project Emergency Response Plan.  Render assistance with the rescue but only from the outside. The Standby person must not enter or lean inside the confined space. The standby person must be clearly identified by wearing a colour vest (high visible vest) and be in possession of a radio, for summoning help, as well as a whistle or air-horn to alert persons inside the confined space. The Standby person must have received appropriate training and be authorized by CONTRACTOR Site HSE Manager to carry out his duties. CONTRACTOR/SUBCONTRACTOR Authorized Gas Tester The Authorized Gas Tester is the person who has been specifically trained and has experience in the usage of the Gas Testing Equipment (flammable / toxic gases, oxygen deficiency equipment etc.). The Authorized Gas Tester(s) is nominated in written by the respective CONTRACTOR/SUBCONTRACTOR HSE Manager. Employees / Authorized Entrants All employees who enter a confined space will:  Review and sign the confined space entry permit  Know space hazards, including information on the mode of exposure (e.g., inhalation or dermal absorption), signs or symptoms, and consequences of the exposure;  Use appropriate personal protective equipment properly (e.g., such as gloves, aprons, and coveralls);  Correctly use all of the required equipment given (i.e., ventilation, means of communication, lighting, PPE)  Maintain communication with the Confined Space Watcher/Standby Person to enable the attendant to monitor entrant status and alert entrants of the need to evacuate the space  Alert the Watcher/Standby person when: o Entrant recognizes any warning sign, symptom of exposure, or the presence of a dangerous situation o A prohibited condition is detected  Exit the space as quickly as possible when: o An order to evacuate is given o The entrant recognizes a warning sign or symptom of exposure to a DOCUMENT TITLE: Confined Space Procedure

PTW SYSTEMS APPLIED

6.1

Restricted/Operational Areas (BROWNFIELD) For activities to be performed within Restricted/Operational Areas (BROWNFIELD) the ADCO PTW System shall be in force as detailed below:  Existing Clusters operational areas  Existing CPP Unit operational area  Existing pipelines and utility corridors (Off-shore and On-shore) operational areas CONTRACTOR and SUBCONTRACTORS will provide to ADCO the relevant WMS(s) and TRA(s) describing the safety protection measures to be put in place when working nearby or on any existing installations that are to remain, compliant with ADCO relevant procedures applied. CONTRACTOR and SUBCONTRACTORS will indicate to ADCO an enough number of Job Performer Supervisors, of Work Permit Coordinator(s) and Job Originator Supervisors to be trained, in order to avoid the delays due to permit processing. As per ADCO PTW procedure, a Confined Space Entry Certificate (CSEC) is required for the protection of personnel entering vessels, tanks, furnaces, sewers, pits over one meter deep and other confined spaces within the ADCO Oil Field/Terminal areas regardless of area designation. The CSEC allows ONLY to enter into a confined area but the work may need additional PTW(s) related to type of work (Cold, Hot, Excavation etc.). Protection is required against hazards such as flammable vapors, oxygen deficiency, toxic atmospheres, excessive temperatures, power driven equipment, etc. Confined Space Entry is one of the most hazardous activities undertaken within petroleum production operations. Care and attention must be paid to all aspects of the work plan and procedure. Particular emphasis must be placed on communication. Since person's lives are directly at risk, pre-job work planning meetings, procedure review meetings, TRA and TBT's are essential pre-requisites. It is recommended that prior to each Confined Space Entry work that all persons familiarize themselves with the Confined Space Entry procedures as detailed in Appendix G to the 3 part of Permit to work – Confined Space Entry Certificate.

6.2

6.3

Non Restricted/Non-Operational Areas handed over to CONTRACTOR (GREENFIELD - OPEN AREAS) For activities to be performed within Non Restricted/Non-Operational areas formally handed-over from ADCO to CONTRACTOR using an ADCO “OPEN AREA Handover Certificate”, the receiving CONTRACTOR CD/Site Manager assumes full responsibility for administration of the OPEN AREA including responsibility for the safe working practices and relevant CONTRACTOR PTW System applied, administration of the PTW(s), review of SUBCONTRACTOR‟s WMS(s) and execution of TRA(s). The CONTRACTOR PTW System shall be in force in such areas as detailed below:  Existing and/or New Clusters Non-Operational areas formally handed-over from ADCO to CONTRACTOR using an ADCO “OPEN AREA Hand-over Certificate”;  New CPP Unit Non-Operational area formally handed-over from ADCO to CONTRACTOR using an ADCO “OPEN AREA Hand-over Certificate”;  New pipelines and utility corridors (Off-shore and On-shore) Non-operational areas formally handed-over from ADCO to CONTRACTOR using an ADCO “OPEN AREA Hand-over Certificate”;  New TCF, Camps and Accommodation Non-operational areas formally handed-over from ADCO to CONTRACTOR using an ADCO “OPEN AREA Hand-over Certificate”. The CONTRACTOR CD/Site Manager is responsible for appointing suitably competent Entry Supervisor to issue and administer the “PTW/ Confined Space Entry Permit” within the OPEN AREA as per CONTRACTOR PTW System applied. The Commissioning Manager (or his delegate) will grant last approval of permits during the Pre- / Commissioning phase inside those areas of his responsibility. The Entry Supervisors will control the issuing of Confined Space Entry Permit within their respective areas of competency (refer to Attachment 1). The Entry Supervisor who signs the Confined Space Entry Permit must be always a CONTRACTOR Discipline / Commissioning Supervisor; he must have personal knowledge of the conditions under which the work will be done, and the hazards involved, and must ensure that the area / equipment is properly checked. CONTRACTOR Entry Supervisor will receive specific Trainings as per “Project HSE Training Matrix” (refer to CONTRACTOR procedure 3844-LZ-PC-GA000101_HSE Training Program). Authorized Gas Tester is the person who has been specifically trained and have experience in the use of Gas Testing Equipment (flammable / toxic gases, oxygen deficiency equipment etc.) and have been nominated by the HSE Manager and Commissioning Manager. CONFINED SPACE ENTRY PROCEDURE

7.1

General An Entry Permit must be posted at all entrances of the confined space or otherwise made available to entrants before workers enter a permit space. An Entry Permit is mandatory for any persons to enter and who intend to perform a job within an enclosed space. The Entry Permit allows ONLY to enter into a confined area but the work may need additional PTW(s) related to type of work (Cold, Hot, Excavation etc.) The Permit must verify that pre-entry preparations outlined in the standard have been completed. The duration of entry permits must not exceed the time required to complete an assignment. Confined spaces include tanks, vessels, hoppers, bins, boilers, conveyor tunnels, coal bunkers, sumps, ducts, scrubbers, manholes, pits, sewers etc. Putting one‟s head inside the manhole or the vessels entrance is also deemed as confined space entry. Because of works to be executed within H2S hazardous classified areas, the Confined Space definition includes excavations below 1.2m (or 4 feet), which are to be considered confined spaces due to the fact that H2S will settle at the bottom of these spaces (hydrogen sulfide is heavier than air). If SUBCONTRACTORS have to execute works within ADCO‟s classified H2S areas, unauthorized entry into pits or trenches shall be prohibited unless specific PTW(s) and TRA(s) have been approved by ADCO and CONTRACTOR, and all mitigation measures assumed by all involved parties. Issuing of ADCO‟s Confined Space Certificate will be required. Entry controls in such areas will be manned by ADCO in terms of accessibility, fencing, lockout/ tag-out and procedures applied. Appropriate signage shall be in place such as the following “Confined Space Danger Sign”:

Area Authority or a CONFINED SPACE ENTRY PERMIT by the CONTRACTOR Entry Supervisor (OPEN AREA), can enter into a Confined Space or act as Safety Watcher/Standby person. A specific WMS and TRA must be approved for each Confined Space Entry and should address the following as a minimum:  Work scope and method.  Supervisor in charge  Watcher/Standby man  Rescue Procedures and Equipment  Training  Tools and Equipment  Explosion proof fittings (if required)  Ventilation  Access/Egress  Bonding to prevent electric shock and static discharge The following activities are considered to be hazardous when carried out in Confined Spaces:  Paint spraying  Welding  Gouging  Preheating and Burning  Grit blasting  Use of Solvents The hazards involved include Fire, explosion, asphyxiation, dust, fumes, noise, electrocution and oxygen deficiency/enrichment. The Confined Space must be positively isolated from all piping connections, either by spades or the removal of valves or spool pieces. All electrical-driven equipment connected to, or normally associated with the confined space, must be locked out for safety reasons. The use of electrical equipment inside Confined Spaces must be strongly discouraged, however, where this is not possible the maximum voltage is 24 volts AC (or 110 volts DC with center tap to earth – this will be subject to additional Permit To Work such as Hot, Energizing etc.). Only air must be used to drive pneumatic tools in confined spaces (except when the atmosphere is inert). The confined space must be prepared and placed in a safe condition for entry prior to the Permit Issuer completing the Hot Work Permit. The actual preparation of an individual confined space will vary according to its design, construction and use.

However, in all cases, adequate lighting, ventilation and safe means of access and egress must be provided. Ensure that torches are available for emergency lighting in case of generator failure. All tools, equipment, cables, tube, ventilation, lighting must enter the confined space from a separate opening. When possible man hole shall be exclusively reserved to men egress/access. Evaluations for heat stress must be carried out by the SUBCONTRACTOR Supervisor and CONTRACTOR Supervisor prior to start of work and at regular intervals, and appropriate action taken. Whenever more than one group of workers is working simultaneously inside a confined space a proper coordination will be organized by CONTRACTOR and relevant SUBCONTRACTORS, the coordination measures shall be reported in the WMS/TRA. No hot work will be allowed inside a confined space until appropriate control measures are introduced, based on the WMS/TRA. The ventilation system, preferably air driven, must be arranged such that only fresh air is drawn or blown into the confined space. In most cases Oxygen and Gas tests must be carried out by CONTRACTOR/SUBCONTRACTOR Authorized Gas Tester or ADCO Area Safety Officer prior to initial entry, and then at intervals determined by the Permit Issuer. (Note: For certain activities continuous monitoring may be required). The Safety Watcher/Standby person must be briefed by the Permit Issuer on his/her responsibilities. An agreed emergency response/rescue plan must be prepared and explained to all persons involved in the confined space entry. The complexity of the plan will be dependent upon the type of confined space; however, all persons entering must wear adequate PPE, including safety harnesses and retrieval lines. Cylinders containing oxygen, acetylene, nitrogen or other gases shall not be taken into confined or enclosed spaces. Internal combustion powered engines shall not be operated in confined spaces. Low voltage transformers must not be taken into confined spaces. When all preparation works are complete the Permit Issuer will sign the Permit to Work which must be displayed at the entrance of the confined space. Each person entering the confined space will attach his/her ID badge to the display board and sign the Entry log sheet, both of which will hang at the entrance to the confined space. Each person leaving the confined space, irrespective of for how long time, he/she must sign the log sheet and remove his/her ID badge. Safety Watcher/Standby person will strictly control entry to the confined space. When there is no one inside, and before he leaves, he/she must either shut the door or place a warning sign to prevent unauthorized entry in his/her absence.

7.2

Prior to Entry Prior to entering in a confined space, a CONFINED SPACE ENTRY CERTIFICATE (CSEC) issued by ADCO Area Authority or a CONFINED SPACE ENTRY PERMIT issued by the CONTRACTOR Entry Supervisor (OPEN AREA) must be properly completed (refer to CONTRACTOR procedure: 3844-LZ-PC-GA000107_PERMIT TO WORK PROCEDURE). “Entry permit” means the written or printed document that is provided by the responsible party (ADCO or CONTRACTOR) to allow activities and control entry into a confined space. The proper protective clothing, respiratory equipment, emergency stand-by equipment, and fire precautions shall be taken before the permitted confined space is entered. CONTRACTOR/SUBCONTRACTOR HSE personnel and SUBCONTRACTOR Inspector should check the equipment if properly inspected, tested and certified prior to be used/introduced into the Confined Space. For further details refer to procedure: 3844-LZ-PC-GA0001020_CONSTRUCTION TOOLS AND EQUIPMENT INSPECTION PROCEDURE. Authorized entrants who enter a permit space must wear a chest or full body harness with a retrieval line attached to the center of their backs near shoulder level or above their heads. Wristlets may be used if the employer can demonstrate that the use of a chest or full body harness is not feasible or creates a greater hazard. Also, the employer must ensure that the other end of the retrieval line is attached to a mechanical device or a fixed point outside the permit space. A mechanical device must be available to retrieve someone from vertical type permit spaces more than five feet (1.524 meters) deep. Specific WMS and associated TRA will define such requirements.

7.3

Permit Emission The CONTRACTOR Entry Supervisor or the ADCO Issuing Authority, for the respective areas of competency, they shall:  Enforce the PTW required measures to prevent prohibited entry.  Distinguish and classify the hazards of a required confined space Permit before employees enter.  Produce and co-ordinate the procedures and operations necessary for a safe permit confined space entry operations and rescue/emergency services may be required. The SUBCONTRACTOR Job Performers shall provide to the employees the necessary equipment indicated in the PTW, at no cost to employees, shall ensure that these equipment are fit to the job and that the employees are using the equipment properly. This includes testing and monitoring equipment, communications equipment, personal protective equipment, lighting equipment, barriers and shields. When there is a change of the task, or conditions stated on the permit change for some reason, the Permit has ceased and a new Permit is required.

7.4

During the activity The CONFINED SPACE ENTRY CERTIFICATE (CSEC) issued by ADCO Area Authority or the CONFINED SPACE ENTRY PERMIT issued by the CONTRACTOR Entry Supervisor (OPEN AREA) shall identify the following:  Name of permit space to be entered, authorized entrant(s), eligible attendants and individuals authorized to be Entry Supervisors  Gas Test results;  Tester‟s initials or signature;  The date and the authorized duration of the entry permit (the duration of entry permits must not exceed the time required to complete an assignment, and in any case cannot exceed 12 hours).  Name and signature of supervisor who authorizes entry;  Purpose of entry and known space hazards;  Measures to be taken to isolate permit spaces and to eliminate or control space hazards;  Name and telephone numbers of rescue and emergency services and means to be used to contact them;  Acceptable entry conditions;  Communication procedures and equipment to maintain contact during entry;  Additional permits, such as for hot work, that have been issued authorizing work in the permit space;  Special equipment and procedures, including personal protective equipment and alarm systems; and  Any other information needed to ensure employee safety  The authorized personnel, by name, currently serving as Standby person. Circumstances may require more than one person posted at the entry point or at different access/entry points. The requirement for additional standby persons should be carefully considered during the planning of the job depending on the specific Task Risk Assessment contents. The permit must be posted at the point of the access/entry. The Entry Supervisor must cancel entry permits when an assignment is completed or when new conditions exist. New conditions must be noted on the cancelled permit and used in revising the permit space program. The standard requires that the SUBCONTRACTOR keep all cancelled entry permits for at least one year. When an unsafe condition arises in or near the required entry permit the CONTRACTOR Entry Supervisor or the ADCO Issuing Authority, for the respective areas of competency, they will stop the work, remove the workers, and revoke the Confined Space Entry Permit.

The CONTRACTOR HSE Manager or ADCO Safety Officer shall retain each cancelled entry permit for recording purpose and to allow the review of the confined space program (Lesson Learnt). Any problems encountered during a confined space operation shall be marked on the appropriate permit so that proper revisions of the permit can be made.

7.5

Worker Training Before the initial work assignment begins, the SUBCONTRACTOR must provide proper training, as per “Project HSE Training Matrix”, for all workers who are required to work in permit spaces. After the training, SUBCONTRACTORS must ensure that the employees have acquired the understanding, knowledge and skills necessary to safely perform their duties. Additional training is required when:  The job duties change;  A change occurs in the permit space program or the permit space operation presents any new hazard; and  An employee‟s job performance shows deficiencies. In addition to this training, rescue team members also require training in CPR and first aid. Employers must certify that this training has been provided. After completion of training, the employer must keep a record of employee training and make it available for inspection by employees and their authorized representatives. The record must include the employee‟s name, the trainer‟s signature or initials and dates of the training. Standby personnel must be trained and shall be assigned no other duty. Under no circumstances should they leave the worksite whilst man-entry in progress. This must be explicitly stated in the TBT. The SUBCONTRACTOR Site HSE Department will document and certify that the trainings required are accomplished. The certification record will include information such as employee's name, the signatures or initials of the trainers, and the dates of training.

7.6

Emergency Situations Emergency situations exist when there is immediate or imminent danger to life and health of the entrants. Emergency procedure will be activated in particular in the following occur:  Failure of monitoring equipment  An emergency occurs inside or outside the confined space The standby person must be notified immediately whenever the entrant notices any warning sign or symptom of exposure to a dangerous situation, or he detects a prohibited condition. If the entrant recognizes any warning sign or symptom of exposure to a dangerous situation, immediate evacuation of the permitted confined space is required. DOCUMENT TITLE: Confined Space Procedure

 If the entrant notices or detects a dangerous situation outside the confined space, then the entrant must evacuate immediately.  Entrants must maintain communication with the standby person, either by radio, verbal, or visual contact.  Entrants must wear a full body harness with a retrieval line secured to the “D” ring placed at the back of the harness and connected to a mechanical device or a fixed point outside the permit space. In case of emergency, where the entrants are unable to physically move out from the enclosed space, the retrieval lines connected to a mechanical device or a fixed point outside the permit space will assist in removing the entrants. If an injured entrant is exposed to a substance for which a Material Safety Data Sheet (MSDS) or other similar written information is required to be kept at the worksite, that MSDS or other written information must be made available to the medical facility personnel treating the exposed entrant. RESCUE SERVICE PERSONNEL SUBCONTRACTORS shall ensure that responders are capable of responding to an emergency in a timely manner. SUBCONTRACTORS must provide rescue service personnel with adequate PPE and rescue equipment, including respirators, and trainings in how to use it. Rescue service personnel also must receive the authorized entrant trainings and be trained to perform assigned rescue duties. All rescuers shall be trained in first aid. At a minimum, one rescue team member must be currently certified in first aid. SUBCONTRACTORS must ensure that practice rescue exercises are performed yearly and that rescue services are provided access to permit spaces so they can practice rescue operations. Rescuers also must be informed of the hazards of the permit space.
