INDIVIDUAL ASSIGNMENT - ACCIDENT INVESTIGATION OBJECTIVE:

Identify ethical principles and commit to professional ethics and responsibilities and norms of engineering practice. [PO8 LO6];{A4}.

ASSIGNMENT GUIDELINES 1. Select a particular Case Study as listed in Appendix 1. 2. Imagine you are safety officer, you received the information regarding the accident as listed in Appendix 1. 3. Part A- You are required to explain each step in order to carry out the accident investigation with professionally ethics, fill the suitable form and prepare the accident report. 4. Part B- Provide suggestions for preventive action, especially in term of engineering control in order to avoid similar accidents happen in the company. (supported with relevant photo/technical diagrams/drawings/ form) 5. Part C- Identify the ethical principles and professional ethics exist/ potential to happen to the case study that you have chosen. Example: construction industry – ethical issues: corruption, materials used are out of specification etc.

REPORT 1. Prepare your report with the details of all the tasks. 2. The report should include part A, B and C. 3. The explanation of accident investigation and preventive action should be supported with relevant photo/technical diagrams/drawings/ form. 4. Provide suggestions for preventive action, especially in term of engineering control in order to avoid similar accidents happen in the company. 5. Identify the ethical principles and professional ethics exist/ potential to happen to the case study that you have chosen. Example: construction industry – ethical issues: corruption, materials used are out of specification etc.
